What is Digital Smile Design and where did it come from?

Digital Smile Design (DSD) is a planning protocol developed in 2009 that uses digital technology - photographs, video, intraoral scans, and specialist software - to design a smile that is built around the patient's whole face, their facial expressions in motion, and their personality. It has been adopted by leading cosmetic dental teams worldwide and is now the gold standard in aesthetic dentistry. DSD changed smile planning from a process driven by the dentist's judgment to one in which the patient co-creates the result and can preview it before committing to any treatment.

Why do people choose Digital Smile Design over conventional cosmetic dentistry?

Because DSD eliminates uncertainty. In conventional cosmetic dental planning, the patient agrees to a treatment based on a description, a wax model, or photographs - and only sees the real result when it is done. With DSD, the patient sees a digital simulation and then a physical mock-up placed on their actual teeth before any treatment begins. They can adjust colour, shape, length, and proportion at the mock-up stage. They know exactly what the result will look like and exactly what it will cost before they commit to anything.

Can any dentist offer Digital Smile Design?

What treatments can be planned through DSD?

DSD is a planning framework, not a single treatment. The specific treatments depend entirely on what the patient's current dental situation requires to achieve the designed smile. The DSD Process

What does the DSD process involve, step by step?

Step 1 - Consultation and data gathering: photographs from multiple angles, video of you talking, smiling, and laughing, intraoral digital scans, and X-rays are captured. A detailed conversation establishes your concerns and goals.

Step 2 - Design and planning centre review: the clinical team designs your smile using DSD software and submits the plan to the international DSD planning centre in Madrid, where specialists from multiple dental disciplines review both the aesthetic and clinical quality.

Step 3 - Smile preview (test drive): you return to the clinic for a physical mock-up of your new smile placed directly on your teeth. You see - and feel - your new smile on your actual face. Adjustments to colour, shape, and proportion are made at this stage.

Step 4 - Treatment begins: once the design is approved and the plan is agreed, treatment begins. The specific procedures depend on your case.

Step 5 - Your new smile: treatment is completed and the final result placed, matched precisely to the digitally designed plan.

Is the process painful?

The DSD consultation and smile design process involves no drilling, no injections, and no discomfort. It has been described by patients as feeling more like a photoshoot than a dental appointment - photographs, video, and digital scans are taken, and questions are asked. The mock-up appointment is equally comfortable - the temporary teeth are placed over the existing teeth without any preparation. The treatment phase that follows will involve the standard level of comfort management appropriate to the specific procedures being performed.

What if I am not sure how I want my teeth to look?

This is not a problem - and it is more common than most patients expect. The DSD process is designed specifically to help patients who do not have a clear picture of what they want. The data gathered at the consultation, combined with the discussion about your concerns and personality, gives the clinical team enough information to propose a design. The mock-up then makes the abstract concrete - you can react to what you see and refine it from there. Most patients find that seeing the mock-up on their face is the moment they know what they want, even if they could not have described it in words before.

What if I do not like the mock-up?

This is exactly what the mock-up appointment is for. If you are not happy with what you see, the design is adjusted. The software allows the clinical team to modify tooth colour, shape, length, and symmetry digitally, and these changes are reflected in a revised mock-up before any treatment is agreed. Nothing is permanent until you approve it. The entire value of the DSD test drive is the ability to refine the design freely, without consequence, until it is right.

How long does the first consultation last?

The initial DSD consultation is not time-constrained at Vilafortuny. It takes as long as it needs to in order to gather all the required data and have a thorough discussion about your goals. Most patients find it takes between 60 and 90 minutes. We do not rush the consultation - the information gathered at this stage is the foundation of the entire smile design, and it needs to be complete.

How should I prepare for the first consultation?

All you need to bring is yourself. Some patients find it helpful to think about what specifically bothers them about their current smile - the colour, the shape of particular teeth, the symmetry - and some bring photographs of smiles they find appealing. Neither is required. If you arrive with no ideas at all, the clinical team will work through the design process with you. The more openly you can discuss what you want and what you do not want, the more precisely the design can reflect your goals.

How much does Digital Smile Design cost?

The cost varies because the treatment plan is specific to each patient - no two DSD cases are identical. What DSD provides, which conventional treatment planning does not, is complete cost transparency before any treatment begins. The full treatment plan - including every procedure and its cost - is presented and agreed at the mock-up appointment before a single permanent change is made. Patients know exactly what they are committing to financially before they commit. There are no additions or surprises during treatment.

How long does DSD treatment take from first consultation to final result?

The design phase - from the first consultation through to mock-up approval - typically takes two appointments. The treatment phase depends entirely on what is required. Cases involving only whitening and bonding may be complete within weeks. Cases incorporating orthodontics, implants, or multiple crowns and veneers will take longer - orthodontic treatment alone may run twelve to eighteen months. The full timeline is established and presented at the mock-up appointment so patients can plan accordingly.

Will the result match what I was shown in the mock-up?

Yes - and this predictability is one of DSD's most important clinical advantages. Because the smile is designed digitally and the laboratory fabricates the final restorations to match the digital design, the result at the end of treatment is reliably close to what was shown in the mock-up.
